It is unlocked by the Feather of the Smoke Red Moon, a Druid-only item that drops from Tindral Sageswift, Seer of the Flame, in the Dragonflight raid Amirdrassil, the Dream's Hope. The drop chance is 0.63% per kill. Once the feather is used, the form appears among the Moonkin options at the barbershop.

No. Each flame form has its own item: the cat comes from Fandral's Flamescythe (Majordomo Staghelm, Firelands) and the bear from the Pollenfused Bristlebruin Fur Sample (Emerald Dream).
