Two patches shaped it: 0.4.0 raised the Life Leech line to 100-200% (it was 100%), and 0.5.0 set the Damage while Leeching line to 25-50%. You pick Softcore or Hardcore and the quantity, up to 15, and the team trades the amulet to your character in game once the details are confirmed in live chat or on Discord.
