How a run plays out

A meter fills while too many enemies stay alive at once, and the run ends when you are Overwhelmed, so fast area damage matters as much as toughness.
Each cleared wave pushes the difficulty up, and later waves can bring several bosses together.
Rewards are handed out when the run ends and scale with how deep it went.
The portal cosmetic hides behind a shrine sequence inside the arena: four shrines in a set order, each within its own tier range, then shadowy Treasure Goblins that grant it with the achievement Only the Mad Run Free.

Where Traces of Echoes come from

Each attempt uses one Trace, a rare loot drop with no fixed source.
Patch 3.1.0 of 30 June 2026 raised the chance for Elites and Champions to drop one.
In Season 15, Keys Caches from Whispers have a small chance to hold a Trace of Echoes or a Mephisto Key.
