How the fight goes

Echo of Akarat: heavy melee, homing Blades of Hatred and the Dark Arrival slam; at 66% of his life his echoes start copying his attacks.
Echo of Mephisto: fire orbs, a stunning Cold beam, a poison tide around him and Lightning that sweeps the arena; below 33% he throws Spines of Hatred without pause.
Hatred's Domain: hands rise from the water, Harvest of Souls ends in a beam, Wave of Defilement locks your skills until you grab purification orbs, and Maw of Hatred kills anyone left in the centre.
Since patch 3.1.0 the mid-fight cutscene can be skipped, and since 3.1.3 a player who joins a fight in progress can take a portal to the third stage.

Loot and rewards

His Hoard does not follow the Lair Boss tables: it holds a large number of Charms, a high chance of Ancestral items and better odds for Mythic gear and Seals.
Patch 3.2.1 (15 September 2026) raised his overall loot quality, and its notes add a Mythic drop to every Echo of Mephisto encounter.
Each player in a party has to open the Hoard separately.

Requirements

The Lord of Hatred campaign completed; the fight is in Skovos, the expansion's region.
Torment X unlocked for the Torment X option, Torment XII for Torment XII.
For Solo Kill (Piloted): a level 70 character whose build can take the fight alone.

Is this the Mephisto from the Season 15 questline?

No. The Hell's Legacy questline has you defeat an echo of Mephisto in the Durance of Hate to win a Splinter of Evil. This lot is the Lord of Hatred pinnacle fight in Skovos, opened with a Crux.

Which kill options are there?

Group Kill (Self-play), Group Kill (Piloted) and Solo Kill (Piloted). The two group options are party kills, and in the game the fight is listed in the Party Finder.
