In Diablo 4 the Pandemonium Event returns as the Echo of Tristram, a hidden Season 15 fight against Diablo, Baal and Mephisto at the same time, built in the spirit of Uber Tristram from Diablo II. Wirt's Lucky Coins, crafted from one Splinter of each Prime Evil, go into the fountain in Old Tristram to open a red portal. The loot of the three includes the Hellfire Torch, a Mythic Unique Charm. The lot sells 1 to 5 completions.

Key facts

In-game name	Echo of Tristram, behind a portal whose name shows as question marks; community name Uber Tristram
Model	The Diablo II Pandemonium Event (Uber Tristram)
Bosses	Diablo, Baal and Mephisto together; the quest objective is Slay the Prime Evils
Key	Wirt's Lucky Coins: stack to 99, Season 15 only, cannot be dropped
Coin recipe	Hidden Horadric Cube recipe: one Splinter of Terror, one of Destruction and one of Hatred
Splinter sources	Echo of Diablo (The Labyrinth), Echo of Baal (Worldstone Keep), Echo of Mephisto (Durance of Hate) and the repeatable seasonal reputation cache
Entrance	A wish at the fountain in Old Tristram on a high Torment tier (exact tier not confirmed); one coin per portal
Access to Tristram	Deckard's Journey: 10 Deckard's Journals, then the Deckard's Memory portal in the Imperial Library
Rewards	Hellfire Torch (Mythic Unique Charm) and the Standard of Heroes trophy
Fountain fix	Hotfix of 22 September 2026: the fountain no longer consumes coins without opening a portal

What you get

Your chosen number of Echo of Tristram completions, from 1 to 5, in Self-play or Piloted mode, on Softcore or Hardcore.
A Hellfire Torch for your Talisman as part of the order.
All loot of the Prime Evils, event resources and the experience of the runs stay with your character.

How the portal opens

Deckard's Journey: all ten Deckard's Journals, after which Deckard's Memory in the Imperial Library leads to Old Tristram.
One Splinter of Terror, Destruction and Hatred. They drop at random from the Echoes of Diablo, Baal and Mephisto in The Labyrinth, Worldstone Keep and the Durance of Hate, and from the repeatable seasonal reputation cache.
A hidden Horadric Cube recipe turns the three into Wirt's Lucky Coins, which stack to 99 and exist only in Season 15.
A wish at the fountain on a high Torment tier takes one coin and opens the portal. The quest inside reads Slay the Prime Evils.

The Hellfire Torch

A Mythic Unique Charm named after the Diablo II charm from the same fight.
Player reports say it counts toward a random class set, so that set needs one charm fewer, and it frees a slot for one more Unique Charm.
Blizzard keeps its stronger-than-intended interaction with several charms unchanged for Season 15.

Note: the hotfix of 22 September 2026 fixed a bug where the fountain took Wirt's Lucky Coins without opening the portal.

Requirements

A Seasonal character: the Splinters and Wirt's Lucky Coins exist only in Season 15.
A high Torment tier: below it, the fountain answers that the minions of Hell are too strong. The exact tier is not confirmed.
Lord of Hatred for the coin craft in the Horadric Cube, the expansion's crafting station.
