Lair Boss Keys are spent after a Lair Boss dies, not before it: since the Season 8 rework anyone can start the fight for free, and a key opens the Hoard that holds the loot. Lord of Hatred folded the old per-boss materials into tiered keys, and patch 3.1.0 renamed Betrayer's Husks to Superior Lair Keys. This lot sells the three ladder keys and the Crux of the False Prophet, the key for Echo of Mephisto.

What each key opens

Normal Lair Boss Keys: the basic Lair Key. One opens the Hoard of an Initiate boss: Lord Zir, Grigoire, Urivar, Echo of Varshan or the Beast in the Ice. Sold from 10 to 500 in steps of 10.
Greater Lair Boss Keys: one per Hoard of Duriel, Andariel, Harbinger of Hatred or the Butcher; also 10 to 500 in steps of 10.
Superior Lair Keys: open the Hoard of Belial, Lord of Lies. They come as key packs, from 5 to 300 in steps of 5.
Crux of the False Prophet - Mephisto Keys: a Crux opens the Hoard of Echo of Mephisto, the Lord of Hatred pinnacle boss fought at Torment X or higher. 10 to 100 in steps of 10.

Note: Greater keys do not open Belial's Hoard: in Season 15 it takes Superior Lair Keys. The Corrupted Reaper was the seasonal Lair Boss of Season 14 and is not on the Season 15 ladder.

Where the keys drop in the game

Lair Keys come from endgame activities such as War Plans, Nightmare Dungeons, Helltides and the Kurast Undercity, and from World Bosses. The Season 15 Season Ranks also hand out Lair Boss Keys.
Greater keys drop from Initiate Lair Bosses, and from Torment VI on they drop more often.
Superior keys come from Belial's ambush, which can follow an Initiate or Greater Hoard, and rarely from the ladder bosses. Baal's Season 15 track adds Colossal Foe at rank 22: Boss Ambushes gain two Monster Power and drop a Superior key.
A Crux of the False Prophet is a rare drop at Torment X and above.

To use the keys: Torment I or higher for Lair Bosses, Torment X or higher for Echo of Mephisto.
