How Varshan works in Season 15

Echo of Varshan is an Initiate Lair Boss in Hawezar and needs Torment I or higher. Summoning him is free; opening his Hoard after the kill costs 1 Lair Key.
The Hoard can drop a Greater Lair Key for the next tier of bosses, and now and then Belial ambushes the player who opened it.
His Boss Trophy is the Malignant Heart. Five of the same trophy amalgamate in the Horadric Cube into a random Unique from his loot table, with a small chance of Mythic quality. In Torment difficulties the Heart also turns up now and then in Nightmare Dungeons, from World Bosses and through Mercenary bartering.

What happened to the old parts

Patch 2.2.0 (Season 8, 29 April 2025) made Lair Bosses free to summon and turned boss materials into keys for the Hoard; an Initiate boss then took 12 of them.
Lord of Hatred folded the per-boss keys into the Lair Key. The Gurgling Head, Blackened Femur and Trembling Hand now state that they have lost their connection to a powerful entity. Alchemists no longer offer the old boss material transmutation, but on Eternal realms they turn pre-expansion boss materials into Lair Keys.
