Berserking on top of Berserking

When you gain Berserking while already Berserk, you become more enraged, which grants increased damage, 6 Fury per second and 10% Cooldown Reduction.

The trigger is a refresh of Berserking before it ends. A build that gains the keyword from several sources keeps hitting the condition, and the helm then converts the overlap into extra damage, resource and faster cooldowns.

Berserking in short

Berserking is a keyword that grants increased damage and increased Movement Speed. The Barbarian gets it from Wrath of the Berserker, which grants it as part of its Ultimate effect, and from items such as the Twin Strikes gloves. The helm always drops with a main stat and Maximum Life.

The helm does not create Berserking on its own. It needs a source of the keyword, and a second source or a refresh to be hit while the first is still running.
