A Helltide takes over a fixed set of connected zones in one region at the top of every hour and lasts 55 minutes, followed by a five-minute break. Inside it, demons drop Aberrant Cinders that open Tortured Gifts, a way to target Legendary items for a chosen gear slot. This lot books 1 to 20 hours of Helltide farming on your character, Seasonal or Eternal, Softcore or Hardcore, with Self-play or Piloted.

Key facts

Schedule	Starts at the top of every hour, lasts 55 minutes, then a 5-minute break
Area	A fixed set of connected zones in one region, e.g. Sarkova Pass, Frigid Expanse, Kor Dragan and Malnok in Fractured Peaks, or Skartara and Celestia in Skovos
Aberrant Cinders	Dropped by monsters and chests; spent on Tortured Gifts
Tortured Gifts	Each is tied to a gear slot and drops Legendary items for it with a chance at a Unique; some drop Forgotten Souls
Threat Meter	Filling it makes you Hell Marked; when it drains, a Hellborne spawns and drops Rare and Legendary items and Lair Keys
Accursed Ritual	Three Baneful Hearts (two before Torment) start the ritual that ends with the Blood Maiden
Torment features	Roaming bosses, meteor showers and slot-specific Gifts appear only in Torment difficulties
Season 15	Diablo's Splinter of Terror modifies Helltide; Flaring Cinders makes Flames of Metus spawn Aberrant Cinders in Helltide
Unlock	Available from Normal difficulty on a new seasonal character

How an hour in the Helltide goes

Kills and chests drop Aberrant Cinders. Each Tortured Gift is tied to a gear slot and holds Legendary items for it, with a chance at a Unique; some also hold Forgotten Souls for the Occultist.
The Threat Meter fills as you fight. At the top you become Hell Marked, and when it drains a Hellborne arrives, dropping Rare and Legendary items and Lair Keys.
Three Baneful Hearts start the Accursed Ritual, which ends with the Blood Maiden.
Roaming bosses, meteor showers and slot-specific Gifts appear in Torment difficulties; before Torment every Gift is a Mystery chest.

Helltide in Season 15

Bearing Diablo's Splinter of Terror changes Helltide and Infernal Hordes.
One of its upgrades makes Flames of Metus spawn Aberrant Cinders inside Helltide, and from rank 18 of its track the Tainted Shrines node makes Helltide shrine effects last 15 seconds longer.
Patch 3.2.1 lowered the chance for Plague Maggot Young of the Hellmouth node to drop Experience Orbs.
