Neathiron is the Unique crafting material that the Blacksmith asks for when you reroll the affix a Masterworked item gained from its capstone upgrade. It used to come from deep inside The Pit; the Masterworking rework of patch 2.5.0 (December 2025) brought it back for capstone rerolls, and today World Bosses, Bartuc in the Infernal Hordes and Belial are where it shows up most.

What Neathiron is spent on

Masterworking raises the Quality of an item step by step and ends with a capstone that upgrades one affix. If the capstone lands on the wrong affix, you can reroll it without losing the Quality you built.
On Ancestral items each reroll costs Neathiron on top of Obducite and gold; two-handed weapons need twice the materials.

Where it drops

Often: World Bosses, Bartuc, Lord of Chaos at the end of the Infernal Hordes, Astaroth in the Escalation dungeons, and Belial, Lord of Lies.
Rarely: Elite monsters and bosses in Nightmare Dungeons.
Bartuc needs Torment I or higher and 666 Burning Aether to face.

Note: the older Neathiron from The Pit is a legacy item; on Eternal realms the Alchemist refines it into Obducite.

No. Quality ranks cost Obducite, gold and salvage; Neathiron only comes in when you reroll the capstone on an Ancestral item.

Is The Pit still a Neathiron source?

No. The Pit version is the legacy item; the material now comes from the bosses listed above.
