In Season 15 the trigger changed. Before, casting Disciple Skills had a Lucky Hit chance to call the spear down. Now consuming Judgement 100 times calls down Spear of the Heavens on a random enemy, and Judgement consumed on Bosses counts as 10. Spear of the Heavens damage is increased by 80-100%.

Affixes and gear details

With Lord of Hatred, Uniques carry fewer preset affixes, which leaves more room to chase a better combination. Since patch 3.1.0 every Unique drops with two fixed affixes, and on Cathedral's Song they are Primary Core Stat and Skill Ranks to Disciple Skills. It is class-locked: only the Paladin can equip it. A Unique Charm version exists, so the same power can also be placed in a Talisman charm slot. Unique items can be tempered, and since patch 3.1.0 Enchanting can alter the affixes of Unique and Mythic Unique items.

Where it comes from

Cathedral's Song is part of the loot table of Urivar, a Lair Boss found in Nahantu, near the Kichuk Waypoint; the boss element type is Fire. The fight needs Torment 1 or higher, and 1x Lair Key opens the Hoard after the kill. The Paladin part of that table also lists Herald's Morningstar.
