An explosion on every cooldown cast

When you cast a Skill with a Cooldown, you explode and deal Fire damage. The damage is a multiple of your weapon damage, so the weapon you wield decides how strong each burst is.

The trigger is the cast and not the hit, so the explosion happens even if the skill itself misses its target. More skills with short cooldowns on the bar means more explosions over the length of a fight.

Which skills have a cooldown

A skill with a cooldown is any skill that cannot be spammed, which covers most Defensive, Mobility and Ultimate skills of every class and a number of class-specific skills besides. Basic and Core skills generally have no cooldown and do not trigger the boots. That makes the item a better fit for builds that lean on cooldown skills than for builds that stand on one spender.

The fixed affixes on the boots, a main stat and an All Damage Multiplier, push the same direction as the explosion: more damage per hit, with no restriction by class. Because the explosion always deals Fire damage, it does not depend on the type of damage your skills deal.

Questions about Rakanoth's Wake

All of them.

Does the explosion need an enemy nearby?

The power triggers on casting the skill and does not name a target condition, so you explode whenever the cast happens.
