Andariel's Visage is a Mythic Unique helm that all eight Diablo 4 classes can wear, and it sits in the Iconic Mythic group, which never drops below Mythic quality. Its Lucky Hit power has up to a 20% chance to release a poison nova that spreads part of the damage you dealt over 5 seconds to enemies in the area. We farm it on your account for Season 15 or for the Eternal realm.

What the helm does
- Unique power: a Lucky Hit effect, so every hit you land rolls the chance, up to 20%, to set off the nova. The nova has no flat damage value: it turns a share of the damage you just dealt into poison over 5 seconds.
- Slot and classes: helm; Barbarian, Druid, Necromancer, Rogue, Sorcerer, Spiritborn, Paladin and Warlock can all equip it.
- Fixed affixes: Primary Core Stat and Poison Resistance on every copy since patch 3.1.0 (30 June 2026).
- Flavor: the item text speaks of the whispers of the Maiden of Anguish, which is Andariel's title.
In June 2023 Blizzard's lead class designer listed it among the six rarest Uniques in the game. It is not a fixed part of the Season 15 endgame builds; the nova simply gets more chances the more often your skills hit.
How it is obtained now
- Any Lair Boss can drop it once you play on Torment I or higher. Patch 3.2.1 gave Initiate and Greater Lair Bosses and Belial higher Mythic drop rates, and the chance for a Mythic grows with the Torment tier.
- No Unique version exists on the live servers, so the Horadric Cube upgrade to Mythic does not apply. The Blacksmith instead has a cache that rolls only Iconic Mythics, priced like the Mythic Unique cache, with a random result.
- The helm is account-bound once it drops.

Andariel's Visage FAQ
Does the nova need poison skills?
No. It triggers on Lucky Hit from any hit and turns part of that damage into poison, so it works on every class.
Can I find a Unique version first and upgrade it?
No. Iconic Mythics stay Mythic-only in Season 15; the Unique versions tried on the PTR were removed before launch.
