Key facts

Mythic quality	Since Season 14 (patch 3.1.0, 30 June 2026) Mythic is an item quality: any Unique can drop as Mythic or be upgraded into one
Iconic Mythics	13 items with no Unique version; they exist only at Mythic quality
All-class Iconic Mythics	Andariel's Visage, Harlequin Crest, Heir of Perdition, Melted Heart of Selig, Ring of Starless Skies, Shroud of False Death, Tyrael's Might
All classes except Spiritborn	Doombringer, El'Druin, Sword of Justice
Class-limited Iconic Mythics	Ahavarion, Spear of Lycander (Druid, Sorcerer); Shattered Vow (Barbarian, Druid, Spiritborn); The Grandfather (Barbarian, Necromancer, Paladin, Warlock); Nesekem, the Herald (Spiritborn)
Horadric Cube route	Upgrade Direct Item to Mythic: one-to-one transfer of a Unique, keeps affixes, Tempering and Masterworking; adds the Crafted tag; not for Unmodifiable items
Jeweler route	Rune Crafting: random Mythic Unique for a chosen slot, no Crafted tag
Blacksmith route	Mythic Unique cache (class-appropriate Mythics, chance at Iconic) and a second cache at the same cost that rolls only Iconic Mythics
Crafted tag limit	Only one Crafted-tag item can be equipped; dropped, Jeweler and Blacksmith Mythics have no tag
Drops	Every Lair Boss can drop Iconic Mythics; since patch 3.2.1 Lair Boss Mythics come from that boss's Unique pool
Enchanting	Since patch 3.1.0 Mythic Unique and Iconic Mythic affixes can be changed through Enchanting
Trading	Mythic Uniques are account-bound and cannot be traded
Lair Boss access	Torment I or higher

Doombringer and El'Druin, Sword of Justice suit all classes except Spiritborn. Ahavarion, Spear of Lycander is for Druid and Sorcerer, Shattered Vow for Barbarian, Druid and Spiritborn, The Grandfather for Barbarian, Necromancer, Paladin and Warlock, and Nesēkēm, the Herald for Spiritborn alone.

How Season 15 makes Mythics

Horadric Cube, Upgrade Direct Item to Mythic: one Unique becomes its Mythic form with affixes, Tempering and Masterworking kept. It cannot take an Unmodifiable item.
Jeweler, Rune Crafting: a random Mythic Unique for the slot you choose.
Blacksmith: a Mythic cache with class-appropriate Mythics and a chance at Iconic ones, plus a second cache at the same cost that rolls Iconic Mythics only.

Note: Mythics made with the Horadric Cube's Upgrade Direct Item to Mythic recipe carry the Crafted tag, and you can equip one Crafted item. Mythics from drops, the Jeweler or the Blacksmith have no tag.

Requirements

A level 70 character on the realm you select.
Torment I unlocked: the Lair Bosses, which can drop every Iconic Mythic, open from that difficulty.
