Where The Title comes from today

The Tenet of Bravery in the Monument of Triumph sells the Guardian Games Weapon Engram for Legendary Marks, and The Title is in it. The engram comes in Tier 1 to Tier 5 versions, and each decrypts into a random event weapon, with a chance of a Holofoil one. Its preview also lists Keraunios, Taraxippos and Triple Laureate.

The Title through the Weapon Perks option: a roll with random perks (1 to 30 rolls), a roll with 1 chosen perk, or a God Roll with 2 chosen perks.
For a chosen perk you tell the pro player which perk you want, for example Closing Time; for the God Roll you name two, for example Closing Time with Swashbuckler.
Weapon Tier: Tier 3, Tier 4 or Tier 5. Higher tiers add perks and enhanced mods; Tier 5 adds special traits.

Yes. Bungie retired Guardian Games in Update 9.7.0 and moved part of its weapons into the Monument of Triumph, where they are sold as engrams for Legendary Marks.

What does a chosen perk mean for The Title?

With a roll with 1 chosen perk the pro player aims for one perk you name, such as Threat Detector; with a God Roll with 2 chosen perks you name two that must sit on the same weapon, such as Threat Detector and Surrounded.
